Transition Services means a coordinated set of activities for a child with a disability that:
•    Is designed to be within a result-oriented-process, that is functional achievement of the child with a disability to facilitate the child’s movement from school to post-school activities, including post-secondary education, vocational education, integrated employment (including support employment), continuing and adult education, adult services, independent living, or community participation;
•    Is based on the individual child’s needs, taking into account the child’s strengths, preferences, and interest; and
•    Including instruction, related services, community experienced, the development of employment and other post-school adult living objectives, and when appropriate, acquisition of daily living skills and functional vocational evaluation.     [602(34)]

Following the Transition Services Procedure Manual should help AAPS providers in documenting transition services in our district.

The Summary of Performance (SOP) is required under the reauthorization of the Individuals with Disabilities Improvement Act of 2004 (IDEA 2004).  “For a child whose disability under special education terminates due to graduation with a regular diploma, or due to exceeding the age of eligibility…”  These are guidelines to help you complete a successful SOP.

WCTC (Washtenaw County Transition Council) Resource Directory is an agency and service directory for students with disabilities from high school and beyond in areas such as transportation, recreation & leisure, adult education, human services and information and referral support groups.

The Transition Guide was developed in collaboration with the Washtenaw County Transition Outcome Project (TOP) team.  The mission of the Washtenaw County Outcomes Project is to ensure that, through local collaborative efforts, every person with disabilities will leave the school system with the necessary skills for transition and opportunity to achieve successful post school outcomes.
